The Ambulatory Care Nurse supports the coordination, integration, and evaluation of nursing practice and clinical policies. This role involves comprehensive patient care, including administering treatments and medications, monitoring for adverse events, providing disease prevention guidance, and offering supportive care. The nurse collaborates with providers to develop patient-centered care plans and participates in quality improvement activities.

Responsibilities:
- Perform physical examinations and diagnostic tests.
- Collect patient health history and record information in electronic health records.
- Provide information, counsel, and education to patients and families.
- Administer medications, including vaccinations and IV therapy.
- Provide comprehensive wound care and perform patient triage.
- Assess and monitor patient responses to treatments and procedures.
- Consult with clinical supervisors and licensed providers to determine treatment plans.
- Delegate and supervise licensed practical nurses, medical assistants, and technicians.
- Support quality initiatives and ensure clinical compliance with patient safety.
- Collaborate with the interdisciplinary team and serve as a resource for other staff.
- Participate in clinical inquiry and promote evidence-based practice.
- Execute medical regimens as prescribed by licensed providers.
- Assess patients, identify unmet needs, and plan comprehensive care.
- Coordinate care with other healthcare providers.
- Collect and document patient clinical data and perform onsite testing.
- Perform medical and nursing treatments according to the plan of care.
- Assist with minor and major procedures.
- Manage clinical in-baskets and ensure timely encounter closures.
- Relay clinical results to patients as directed by the provider.
- Perform in-person and phone triage for patient clinical concerns.
- Identify patient, family, and community deviations from health and wellbeing.
- Develop patient-centered plans of care and educate patients and families.
Required Skills & Certifications:
- Minimum 2 years of experience
- Active Registered Nurse (RN) license in New York (NY)
- BLS certification
- ACLS certification
- Experience with EPIC EMR
