Job Summary

As an RN ESU Triage Nurse, your primary mission is to triage patients to determine priority for visits and provide vital education and instruction over the phone or in person as required. You will coordinate after-hours admissions and ensure seamless communication with the interdisciplinary team (IDT), patients, families, and referring agencies to maintain continuity of care after hours.

Key Responsibilities

  • Triage & Assessment: Thoroughly listen to callers, assess physical and psychosocial needs, and use established protocols to assign visits based on priority.
  • Responsive Care: Respond to all calls in a timely and courteous manner, ensuring callbacks are made within 20 minutes of receiving a report from the answering service.
  • Clinical Direction: Provide guidance to the after-hours team and coordinate care with IDT members, physicians, and directors to ensure high-quality service.
  • Documentation: Implement and document the plan of care via email and clinical records to ensure all interventions are accurately tracked.
  • Patient Education: Stay current on triage protocols and medication formularies to effectively educate patients and families.

Qualifications

  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current Texas State license as a Registered Nurse and a current Texas Driver’s License.
  • Experience: One (1) year of experience as a Registered Nurse in a clinical care setting is required; hospice experience is preferred.
  • Skills: Strong nursing skills, excellent interpersonal abilities, and the ability to read and write English fluently.
  • Requirements: Valid Texas Driver’s License, reliable transportation, and auto liability insurance.
    • Ability to work a flexible schedule and travel locally.
    • Proof of current CPR certification, TB testing, and Hepatitis consent/declination.

Working Conditions

This position involves working in patients' homes and facilities in various conditions, including occasional exposure to infectious diseases and unpleasant weather. The role requires prolonged sitting to field calls, the ability to lift up to 20 lbs, and significant hand-eye coordination. You must be highly organized to manage high call volumes and make quick resource acquisition decisions under stressful conditions.
