Job Summary:

The Physical Therapy Assistant provides therapy services within a Skilled Nursing Facility (SNF) setting, supporting patient rehabilitation and recovery. The role involves delivering therapeutic interventions under the supervision of a physical therapist, documenting patient progress, and collaborating with the care team to optimize patient outcomes. Candidates should have relevant experience and hold an active California professional license.

Responsibilities:

• Provide physical therapy services to patients in a Skilled Nursing Facility setting.
• Assist in implementing treatment plans developed by supervising physical therapists.
• Document patient care and progress accurately using appropriate software, preferably NetHealth (Rehab Optima).
• Collaborate with interdisciplinary team members to coordinate patient care.
• Maintain patient safety and ensure compliance with facility policies and procedures.

Required Skills & Certifications:

• Active and current Physical Therapy Assistant license in the state of California.
• Minimum of 1 year of relevant experience, preferably in a Skilled Nursing Facility setting.
• Proficiency in documentation and workflow software; experience with NetHealth (Rehab Optima) preferred.

Preferred Skills & Certifications:

• Prior experience working in a Skilled Nursing Facility environment.
• Strong communication and interpersonal skills to effectively interact with patients and care teams.

Special Considerations:

• Work performed primarily in a Skilled Nursing Facility setting.
• Compliance with state licensing and facility regulations is mandatory.

Scheduling:

• Work schedule and shifts to be determined by facility needs.
